For this week utilize the tool listed above to complete the assignment.
Overview  As you will learn throughout the program, the diagnosis of a variety of psychiatric illnesses is not always an easy or straightforward process. Multiple observations and assessment methods are often employed to reach a diagnosis. This approach can include the use of standardized assessment instruments.  This then aids you in defining a treatment plan and choosing specific treatment plans to use in the care of your clients. You are tasked with identifying a standardized assessment instrument/tool to measure the disorders listed for each week. You will keep these instruments in the form of a “portfolio” that you can use in your clinical practice to assess clients who present with a variety of symptoms. Instructions: Instrument/ Tool criteria: For each assessment, you are tasked with selecting, you will identify an instrument and: List what DSM diagnosis the tool/instrument is used for. Identify an assessment/diagnosis instrument. Appraise a scholarly, peer-reviewed article that addresses the use of the instrument to support your choice as an evidence-based instrument for practice. Evaluate the instrument’s appropriateness for diagnosing the condition it is designed to assess or if the developers of the instrument reported that the instrument is only part of a comprehensive assessment for the disorder. Describe whether or not the instrument can be used to measure patient response to therapy/treatment or if it is strictly for assessment and diagnosis. Discuss the psychometrics/scoring of the instrument, including reliability and validity. Discuss any limitations associated with the use of the instrument. Include a link to view the assessment if possible. Use the following template in completing your portfolio assignments. Your information can be in a bulleted format or just a couple of sentences for each criterion listed above. However, you must use APA citations.  You are NOT required to write this in a paper format.  Turn in one document for each week’s topics.  (However, create a file on your desktop to compile your portfolio as you move through the term.)  This will ensure you can have easy access to show the full portfolio and once you begin clinicals and practice. Throughout the program, you will continue to add to the portfolio in each course.

Here’s a structured template to complete your portfolio entry for Suicidal Ideation Suicide Assessment – C-SSRS following the assignment requirements. I’ll provide a detailed outline that you can customize based on your findings.

Suicidal Ideation Suicide Assessment – C-SSRS

1. DSM Diagnosis the Tool is Used For

  • The Columbia-Suicide Severity Rating Scale (C-SSRS) is used for assessing suicidal ideation and behavior.
  • Relevant DSM-5 diagnoses include:
    • Major Depressive Disorder (MDD)
    • Bipolar Disorder
    • Post-Traumatic Stress Disorder (PTSD)
    • Borderline Personality Disorder (BPD)
    • Schizophrenia and Psychotic Disorders

2. Identification of the Assessment/Diagnosis Instrument

  • Instrument Name: Columbia-Suicide Severity Rating Scale (C-SSRS)
  • Developer: Columbia Lighthouse Project
  • Purpose: To assess the severity and intensity of suicidal ideation and behaviors in clinical and non-clinical settings.

3. Scholarly, Peer-Reviewed Article Supporting the Instrument

📖 Citation Example (APA format):
Posner, K., Brent, D., Lucas, C., Gould, M., Stanley, B., Brown, G., Fisher, P., Zelazny, J., Burke, A., Oquendo, M., & Mann, J. (2011). The Columbia-Suicide Severity Rating Scale: Initial validity and internal consistency findings. The American Journal of Psychiatry, 168(12), 1266-1277. https://doi.org/10.1176/appi.ajp.2011.10111704

Summary of Findings:

  • The C-SSRS is validated as a reliable and evidence-based tool for assessing suicidal risk.
  • The study found high internal consistency and predictive validity in assessing suicidality across various populations.
  • It is widely used in clinical trials, psychiatric evaluations, and emergency settings.

4. Appropriateness for Diagnosis

  • The C-SSRS is NOT a standalone diagnostic tool but plays a crucial role in identifying suicidal risk levels.
  • The developers recommend using it as part of a comprehensive suicide risk assessment that includes clinical judgment, patient history, and other diagnostic measures.

5. Use for Measuring Patient Response to Therapy/Treatment

Yes – The C-SSRS can be used to track changes in suicidal ideation and behavior over time.

  • It is routinely used in clinical settings to monitor the effectiveness of interventions and treatment plans.

6. Psychometrics/Scoring (Reliability and Validity)

  • Reliability:
    • The C-SSRS has strong inter-rater reliability, with studies showing high agreement among different raters.
  • Validity:
    • Predictive validity: Accurately identifies individuals at risk for suicide attempts.
    • Concurrent validity: Correlates well with other established suicide risk measures.

📌 Scoring Method:

  • The scale assesses suicidal ideation (five categories) and suicidal behaviors (actual attempts, preparatory behavior, and non-suicidal self-injury).
  • Higher scores indicate a greater risk of suicide.

7. Limitations of the Instrument

Potential Limitations:

  • Self-report bias – Patients may underreport suicidal thoughts due to fear of hospitalization or stigma.
  • Not a diagnostic tool – Should be used alongside clinical evaluation.
  • Cultural considerations – May need adaptation for different populations and languages.
